The six chest leads are measured by connecting the positive input of the instrumentation amplifier to the six chest electrodes where the negative input is always connected via resistors to the limbs which were used to measure Einthoven I,II,III.
Note that we do not mention N/GND in this clip but it is still connected to the right leg as shown.
